Patient Information
Those wishing to attend our practice may apply by filling in the form, as below, and submitting it to the practice. If you have previously attended only for a covid vaccination, you will need to apply too to join the practice for all other ongoing care.
If you are accepted as a new patient ,you may assist us to get medical information to be sent to this practice by filling out the medical release form on this website , and sending back to us to send on. Receiving medical information from your previous practice in an XML format if that practice used Best Practice software will greatly enhance your chances of becoming a patient of this practice.

If you wish to request a script please book through Hot Doc portal and click ‘Repeat Prescription’. QR code will take you to the Hot Doc booking page.
